James Van Der Beek Opens Up About His Stage 3 Cancer Journey and Family Support

James Van Der Beek has openly discussed his fluctuating health journey ever since revealing his colorectal cancer diagnosis in November 2024.

“Roughly two billion individuals globally are diagnosed with this condition annually, and I am among them,” the former Dawson’s Creek star disclosed in an Instagram update then, mentioning his positive outlook and sense of strength during his ongoing therapy.

The American Cancer Society indicates that colorectal cancer originates in either the colon or rectum, with its specific name—colon or rectal cancer—determined by the location of its development.

Van Der Beek elaborated further on his diagnosis in a November 2024 conversation with People. “I’ve been privately managing this health issue and taking measures to address it, supported by my incredible family,” he informed the publication, noting his cancer was already at stage 3. “There’s cause for hope, and I’m feeling well.”

Upon Receiving the Diagnosis

In a November 2024 video interview with People, Van Der Beek recounted undergoing a colonoscopy in the summer of 2023 after noticing certain bodily changes. “As I was emerging from the sedation, the gastroenterologist, with his most pleasant bedside manner, stated, ‘It is cancer,’” he shared. “And I believe I entered a state of shock for a moment.”

Van Der Beek likened living with cancer to having a “full-time occupation,” involving everything from scheduling appointments, tracking test results, and managing insurance. Shortly after his colonoscopy, the actor learned his cancer was “localized” but had reached stage 3.

“I genuinely didn’t feel this would be my end. I truly felt, ‘This is going to be the most significant life redirection. I’m going to implement changes I never would have made otherwise, which I will reflect upon in a year, five years, 30 years from now and say, ‘Thank goodness that occurred,’” he expressed. “And that was my mindset about 90 percent of the time. Then, 10 percent of the time, I was a weeping, terrified mess.”

Family as Priority

“My children have been so supportive, so loving, and so gentle,” Van Der Beek shared in his November 2024 People video interview. “And it’s tough to witness them needing to mature in that manner, but you see them genuinely prioritize you, which, as a father, is both heartwarming and heartbreaking.”

Van Der Beek mentioned that although his children are “frightened” for his well-being, his diagnosis has also presented “a genuine chance for connection, to be present with them and deeply engage, asking them to voice their thoughts, their fears [and] their concerns.”

He then quipped, “I’ve asked every doctor I’ve seen, ‘What are the chances—and be truthful with me—that I can get my kids to stop calling it butt cancer?’ And I must say, the prognosis on that one is never favorable.”

In his People interview, Van Der Beek informed the publication he’s “cautiously optimistic” about his recovery from cancer. “Once I’m completely in the clear for what feels like a sensible duration, I’ll get back to you and let you know,” he said. “I have a great deal to live for.”

He also noted that his diagnosis provided him with a newfound awareness of his body. “If you’ve heard of it, I’ve probably explored it,” he asserted. “This has been an intensive lesson in mastering mind, body, and spirit. I thought, ‘This will either remove me from my body, or it will teach me how to truly inhabit it.’”

Devoted Partner

While reflecting on his “challenging year” in a November 2024 Instagram post, Van Der Beek lauded the unwavering support he received from Kimberly. “I’m more thankful than ever for my superhuman wife who has appeared on levels I never imagined possible,” he penned alongside a touching family photograph. “Who has shown me what unconditional love is and the magic that stems from that. I am in awe of you, @vanderkimberly.”

That same month, Van Der Beek told People he couldn’t “get through this” without his wife. “And as much as it was difficult to be me this past year … there’s not a single moment I would have ever wanted to exchange with her and have her endure watching me go through that. She is extraordinary.”

Fundraising Efforts

Van Der Beek offered limited editions of his Varsity Blues jersey for sale to gather funds for families affected by cancer. “100% of my net proceeds will aid families recovering from the financial strain of cancer (including my own ),” he captioned the since-removed November 2024 Instagram post.

Promoting Awareness

Van Der Beek was among several celebrities who disrobed to raise money for cancer testing and research during Fox’s December 2024 The Real Full Monty special. “I discovered just how much I truly value having a good group to not navigate something on your own,” he exclusively told Us Weekly about working on the project. “I used to be quite solitary and simply wouldn’t ask for assistance. And participating in this group truly made me realize just how crucial that is and how much I valued it.”

He also emphasized the importance of getting screened and discussing one's health with a medical professional. “Any stigma that might surround it is trivial compared to what could occur if you don’t get tested. So go out and do it,” Van Der Beek urged Us.

Emotional Burden

Van Der Beek stated that his physical health was “exceptionally good” during a December 2024 appearance on Good Morning America, but disclosed his mental well-being suffered. “Emotionally, you know, it’s a lot. It’s genuinely an emotional rollercoaster,” he said on the ABC morning program.

Remarking that he was “stunned” after receiving his diagnosis, he continued, “I had an entire plan for what I envisioned my year would be and what I thought my priorities would be … and the realization that all of that was going to change and take a different course—it felt like a nightmare, honestly, initially.”

Van Der Beek commended his father, James, for being a pillar of support throughout his cancer journey while extending Happy Birthday wishes to his dad via Instagram in January 2025. “The way you have been there when I’ve been utterly down and out this year—without us needing to ask, without the expectation of even a thank you… has been not merely life-saving, but life-affirming,” he captioned his post. “Moving onto the ranch when I’m away for treatment. Preparing lunches, making fires for @vanderkimberly, chauffeuring kids, even tending to the dogs and chickens (neither of which you ever desired and would just as soon never have had ).”

He went on to express that he’s “astonished” by how his dad has “been there for me emotionally as I’ve navigated everything cancer brings up,” adding, “I know it’s beyond your comfort zone, but especially since Mom passed, you have consistently come through for me in ways I didn’t even realize how much I needed.” (Van Der Beek’s mother, Melinda, passed away at age 70 in July 2020.)

A Near Miss

Van Der Beek celebrated his 48th birthday in March 2025 by sharing insights gained from his cancer experience thus far. “And then this year, I had to confront my own mortality directly. I came face-to-face with death,” he stated in an Instagram video. “All of those definitions I held so deeply were stripped away from me. I was undergoing treatment elsewhere, so I could no longer be a helpful husband to my wife. I could no longer be a father who could lift his children, tuck them into bed, and be there for them.”

The CSI: Cyber alumnus mentioned he was “faced with the question that if I was just here to be a too-skinny, weak man, alone, in an apartment with cancer, what am I?” Fortunately, he found comfort in self-acceptance. “I meditated, and the answer emerged. I am worthy of God’s love, simply because I exist. And if I’m worthy of God’s love, shouldn’t I be worthy of my own?” he conveyed to fans.

Openness With His Children

Van Der Beek discussed how he and his wife explained his cancer diagnosis to their kids during a July 2025 appearance on Today.

“I’m far from an authority, but our method was simply to be as truthful as possible, truthful to the extent of their comprehension, right? Because they perceive,” he stated, adding that his children can “sense that Dad’s having a difficult day.”

“They know if Dad is experiencing discomfort. They know, and so by not informing them, I believe you’re causing them even more confusion,” Van Der Beek continued.

Van Der Beek observed that children's resilience can be surprising and urged other parents in similar situations to be honest about their journeys.

“When you communicate what you’re doing and you explain the approach, they can observe it and they can feel it, and I think it’s also, your experience is their experience,” he clarified. “I have kids preparing tea for me. ‘Dad, what do you need?’ A lot of positive outcomes have resulted from it, but I would love to spare everyone this journey, which is why I’m advocating for screening.”

Unexpected Diagnosis

In a July 2025 interview with Today.com, Van Der Beek encouraged others to adhere to colon cancer screening recommendations.

“Even just the slightest alteration, it could signify something, but … don’t assume that lacking symptoms means you don’t need to get screened, especially for a condition that is so treatable when detected early,” he remarked. “That’s truly what I aim to convey. I ate as healthily as possible. I was fit. I was in excellent cardiovascular condition. There was no rationale in my mind that I should have received a positive diagnosis.”

Missing the ‘Dawson’s Creek’ Reunion — To a Degree

Van Der Beek was scheduled to participate in the live reading of the Dawson’s Creek pilot at the Richard Rodgers Theatre on Broadway on September 22, 2025, an event organized to raise money for F Cancer. However, he announced via Instagram on September 21 that he would be unable to attend in person alongside his former castmates due to illness.

“This is the evening I’d been anticipating THE MOST since my wonderful Michelle Williams mentioned organizing it, way back in January… So you can imagine how devastated I was when two stomach viruses conspired to incapacitate me and keep me confined at the worst possible moment,” he wrote. “Despite every effort … I won’t get to be there. I won’t get to stand on that stage and thank every person in the theater for supporting me, and opposing cancer, when I needed it most.”

Van Der Beek was replaced by Lin-Manuel Miranda in the role of Dawson.

He continued, “But I DO have an understudy. A remarkably overqualified substitute who would have been #1 on my wish list (had I ever believed he’d be available). Someone my children would certainly consider an improvement over me. Everyone please relish all the affection in that room. Share some with my family. I will be sending and receiving positive energy from afar in a bed in Austin.”

Van Der Beek ultimately surprised fans by making a virtual appearance via a prerecorded message played at the commencement of the event.

“I cannot believe I am not present,” he stated. “I cannot believe I don’t get to embrace my castmates, my wonderful cast, in person, and I simply wanted to stand on that stage and thank every single individual in this theater for being here tonight—from the cast to the crew to everyone who volunteered their time and has been so generous. And especially every single one of you. You are the best fans in the world. Thank you for coming.”
